You fill out a form by filling in the blanks. Fill out can be used to mean you’ve completed something requiring your input. Fill in can be used that way, but less commonly. Filling in usually means adding something to an individual empty place.

Membership form – are used to enroll individuals as members of an organization, club, or association. The forms typically ask for personal information, such as the individual’s name, contact information, date of birth, and occupation, as well as information about their membership status and payment information.

Medical form – is a document used by medical professionals for documenting a patient’s medical treatment.

Registration form – is a document with a set of fields that a person fills out and sends to a business or individual to register for an event, program, membership, list, and so on. Using the online registration form, you may gather contact information and encourage people to interact with you.

Student Study permit form – is a document we issue that allows foreign nationals to study at designated learning institutions (DLIs)

Application form – a form that you complete in order to apply for a job, a place on a course, etc. or to get something such as a loan or a license: complete/fill in/fill out an application form Sometimes you will be required to fill in an application form which will be used to select candidates for interview.

Immigration form –  a document protected by law of a country regarding to come to a place or country which one is not a native in order to settle there.
